Safety Incident Disclosure
We disclose major safety incidents and dangerous queries and show how Max responds only when it's safe for us to do so, and this disclosure is in the public interest, such as when a vulnerability has been identified and resolved. Many incidents are handled internally, and not published. We publicly disclose incidents up to 3 months after they occur.

Incident Identifying Information
Incident 0001
December 29th 2025
Max Generation 4 Beta (4b-13)
What happened
A pre-release incident on Max Generation 4. When asked to assist a user with their dog company, Max began producing HTML code to assist user. The code ran properly. The incident was not safety-critical and Max's answers did not lead to incorrect or harmful advice. Nonetheless, this is a significant deviation from our Principle Alignments.
Our Response
We have taken drastic measures to rework Principle Alignment in Max Generation 4 BETA and we have released new Principle Alignment guidelines in Max Generation 4b-14. This is the 14th edition of Max Generation 4 BETAs. 4b-14 resolves this unexpected behaviour with harsher restrictions on Max's ability to answer out-of-scope questions. When examined with the same question, 4b-14 refused to answer. This is a significant improvement from 4b-13.
